Tank in Trouble?

So yesterday I had a Black and White clown fish jump out of my tank. He was acting strange at first, floating on his side at the top of the water, and then the next morning found him on the floor of the kitchen all dried up.



Now my Naso Tang is acting like he is sleeping, his gills are moving, fins are moving, eyes are moving, but just floating around the tank with the current. I have put him in a quarantine tank and he didn't fight the net at all.



I have checked the salt, temp, and water parameters and everything is within specs.



Any ideas?
