Everything Dies, Parameters are in check.. HELP!

Hi everyone,

My name is John and I'm new to this forum. I hope I am posting according the rules and in the right section and if not i apologize. I have been into saltwater tanks for quite some time on and off. I previously had tank containing a couple hardy corals, and anemone and various inverts including an above tank refugium FILLED with amphipods and copepods.

I just started a nano reef tank, using the fluval Spec V, 5 gal aquarium setup. I added about 2-3 lbs of "live sand" (the larger pebble sized substrate, not the fine live sand with lots of water in the bag, this bag had little to no water in it) and about 3-5lb of rock (about half live rock, half dried rock) Link to the tank:


I allowed the tank to cycle for a little less than a month, putting food into the tank without being stocked at all. I then had the water tested at the LFS and they said everything was good to go and safe for fish. (Not sure exactly what tests were ran).

Well i bought a couple snails and a couple hermits, all them died the first night. I began to think what may have happened, is that "purified" water was added to the tank by mistake. I did a couple water 40% changes over the next month (using only distilled water) and continued with the food occasionally. Took the water into the LFS was again assured the parameters were all good. Bought a couple more Snails and hermits and even a small clown. They all died within 3 days...... I don't know what to do from here. Here are the only thoughts i have about what the possible cause for this problem may be:

-Purified water?
-Some well water was used at first (I used this water with my old setup)
-When the tank was first setup i failed to rinse the glass/filtration media very well (if at all)
-I took out the stock filter setup (it used a sponge, which i replaced with a rack to hold the filtration media.
-The tank had relatively low flow, i was planning to add a powerhead.

Please help if you have any idea what the problem may be..... I'm in over my head here and I've already sunk quite a few $$ into this setup and i feel terrible about killing that poor clown (first fish that has ever died under my care as an adult :( ) Any ideas anyone?
