This is in a 55 gallon tank which has been going for about 10 months. The spots are mostly visible on the glass but are also on rocks, snail shell, etc. They are tough to scrape off, usually takes a few passes with a magnetic scrapper.
I attached a picture (not the best camera). You can see some on the snail shell and some on the glass under the snails foot.
