What is wrong with my condy anemone?

I'm concerned about my anemone because it has shrunk and doesn't show it tentacles anymore. I bought it a few weeks ago on Nov.28 but after I did a water change it never came back out. It's been over a week and now isn't attached to anything and looks like its dying. Here is a picture http://ift.tt/1AgeJcu
