I think my Condy Anemone is dying...

The past few weeks my Condy anemone has been hiding, moving about the tank and not taking food. The tank is about 9 months old and ive had the Condy in it for about 5 months or more. It's a 10 gal nano tank. (Yes, I know it's a small tank) It stays very small and doesn't open up or extend its tentacles. My levels are good. Salinity 1.025, nitrate is about 10, calcium is a little high about 500ppm and amonia is 0. Am I about to lose my Condy? :(
